In the game Hotaru and I play Baren and Okuni as having a very physical relationship, and since they're twins, this can be a trigger for some. Please leave a comment with your character's names, work names and user names on this post to opt out of mentions of that.

They themselves know that their relationship isn't what siblings should have (at least Okuni does, she doesn't speak for Baren), and it'll probably end at some point when they're in a better state mentally.
